BASELINE ASSESSMENT OF PRIMARY HEALTH FACILITIES IN ASSAM AND MADHYA PRADESH – MRF – 130
 
I.          INTRODUCTION & BACKGROUND
 
LEHS, a not-for-profit organization works towards improving the quality and access to primary healthcare services for underserved communities. The Organization is working on primary healthcare models through a partnership with State governments, international agencies, and the private sector by appropriately introducing promising healthcare innovations in the public health system in order to build an equitable, innovation-led, and evidence-based quality healthcare system within easy reach of the underserved population of Rajasthan, Madhya Pradesh, Delhi, Uttar Pradesh, Maharashtra and Assam. WISH (Wadhwani Initiative for Sustainable Healthcare) is a flagship program of LEHS in India.
 
LEHS is playing the role of a facilitator in setting up and operationalizing digital Health and Wellness Centers in alignment with the Government of India’s vision of provisioning comprehensive healthcare services at the primary healthcare facilities (SCs and RPHCs, UPHCs). The operational diversity of the model of Health and Wellness Centers across states has elements comprising Teleconsultation, Point of care devices for diagnostics EHR and Automatic Drug Vending machines. One such model will include the strengthening of a First Referral Unit (FRU) to provide specialist teleconsultations. In order to upgrade the existing tea garden hospitals of Assam and the urban primary health centre (UPHC) to DHWCs, a holistic assessment of the readiness of the facilities is essential. In this context, a detailed facility-based gap/readiness assessment is planned to be conducted in 10 tea garden hospitals across three districts (Nagaon, Golaghat, and Dibrugarh) of Assam and 8 UPHCs in Jabalpur district of Madhya Pradesh along with community assessment to understand the behavior & knowledge of the community. 8 UPHCs in Jabalpur district (Madhya Pradesh) and 10 Tea Garden Hospitals (TGH)/Dispensaries across 4 districts of Upper Assam, namely; Dibrugarh, Charaideo, Golaghat, and Nagaon were selected after a detailed need assessment. As the Tea Garden hospital and UPHC Model of dHWC has been initiated with the completion of the installation of the equipment in the hospitals/dispensaries and after ensuring facility readiness for operationalization, LEHS|WISH has planned to conduct a baseline assessment of these facilities along with the community assessment it caters to in order to understand health status, health seeking behavior and the service availability.
 
Also, planning to develop these facilities into model Resilient Health Facilities. During any emergency (like COVID-19), a resilient health facility should be able to effectively adapt in response to dynamic situations and reduce vulnerability across and beyond the system. Health facility resilience should focus not only on absorbing unforeseen shocks precipitated by emerging health needs (like COVID-19) but also on ensuring continuity in health services but also fostering people-centeredness while delivering high-quality care. Considering the risk of another wave, the national and state Government has started prioritizing efforts on building health systems resilience and emergency preparedness to face a similar situation in the future (including decentralized public health surveillance systems). In sync with the need, Government priority, LEHS will work together with two State Governments of Assam and MP in developing Model Resilient Health Facility. These facilities will not only be equipped to handle any emergency in the future (especially infections like COVID) but also serve as a ‘model site’ for other facilities to visit and learn the processes and systems.  
 
II.         PURPOSE OF THE STUDY

The objective of the study encapsulates.
To understand the gaps in Infrastructure, HR and training, Drugs and supplies, equipment, and data management systems at the tea garden hospitals.
To assess the service availability and readiness of the facilities to deliver the twelve services as envisaged in HWC guidelines in these tea garden hospitals. (This will be assessed through a facility gap assessment tool.)
To understand, assess and analyze the KAP / behaviors of communities on health care seeking (specifically hypertension, Diabetics & anemia above 18 years) as well as the awareness of affirmative action pertinent to health across HHs keeping vulnerabilities in mind.
Understanding enablers and disablers of the community health ecosystem in the tea garden area thereby providing insights for informed decision-making.
III.        GEOGRAPHY OF THE STUDY
 
The assessment will be conducted in 10 tea garden hospitals across four districts (Nagaon (5), Golaghat (1), Charaideo (1) and Dibrugarh (3)) of Assam and 8 UPHCs in Jabalpur district of Madhya Pradesh.
